INDICATION

Valtec-AMH 10+160+12.5mg Tab is a triple fixed-dose combination antihypertensive indicated for the management of hypertension in adults whose blood pressure cannot be adequately controlled on a dual-combination regimen. MECHANISM OF ACTION

Valtec-AMH achieves blood pressure reduction through three distinct and complementary pathways. Amlodipine, a calcium channel blocker, relaxes the smooth muscle cells of peripheral blood vessels by blocking calcium ion entry, reducing vascular resistance and lowering systolic and diastolic pressure. Valsartan, an angiotensin II receptor blocker, prevents angiotensin II from binding to its AT1 receptor on blood vessels and the adrenal cortex, inhibiting vasoconstriction and aldosterone-mediated sodium and water retention. Hydrochlorothiazide is a thiazide diuretic that acts on the distal convoluted tubule of the kidney to inhibit sodium and chloride reabsorption, increasing urinary excretion of salt and water and thereby reducing blood volume and cardiac preload. The combined action of all three agents provides a powerful, well-tolerated, and sustained reduction in blood pressure across the 24-hour dosing interval.

DOSAGE AND ADMINISTRATION

DRUG INTERACTIONS

Concurrent use with other antihypertensives, nitrates, or vasodilators may produce additive hypotension. Potassium-depleting effects of hydrochlorothiazide are worsened by corticosteroids, ACTH, and amphotericin B, increasing the risk of hypokalemia. Hypokalemia induced by this combination increases the cardiotoxic risk of digoxin and requires close monitoring. NSAIDs may blunt the diuretic and antihypertensive effects. Lithium clearance is reduced by thiazide diuretics, increasing toxicity risk. Iodinated contrast agents used in radiology may precipitate acute kidney injury in patients on this regimen.
